I had the "Offer to create birth facts..." box ticked but the "...even if existing birth includes place and date" box was not ticked.

Would this be the cause of what I was seeing?

I agree with Maxi on this subject - the V7 Descendants Outline report is a major step down from the V6.2 version. Any report must be 'user centric' in that it is written and structured to meet the needs of the reader (who is always the primary user of any report). The V6.2 version was a good example...
